Description

Naturally produced mostly in the ovaries (with a small amount from the adrenal glands), progesterone is a critical hormone. It's necessary for regulating menstruation, sustaining pregnancy, and keeping estrogen levels in check.

ACTIVE INGREDIENTS

Micronized Progesterone

  • Peanut-free compounded product

HOW TO USE

Progesterone Capsule

Dosage will be determined according to the patient's lab values and clinical presentation. It is recommended that the capsules be taken at bedtime to prevent daytime drowsiness.

Progesterone Cream

Once a day, usually in the morning, rub a small amount of the medication onto clean, dry skin. Good places to apply it are your inner arms or inner thighs. Choose a spot where your clothing won't rub against it.

Progesterone Troches

Place one troche between your cheek and gum and let it dissolve completely. Do not chew or swallow it. Use this medication exactly as prescribed by your healthcare provider.

How It Works?

Progesterone is a potent hormone that affects multiple systems in the body through two main mechanisms: a systemic action via cellular receptors and a targeted, relaxing effect in the brain.

1. Cellular and Systemic Effects

Progesterone primarily acts by binding to intracellular progesterone receptors inside target cells (such as those in the uterus, breast, and brain). Once bound, this receptor-hormone complex moves into the cell nucleus, where it functions as a transcription factor to regulate gene expression.

This systemic action is responsible for:

  • Endometrial Maturation: Preparing the uterine lining for potential pregnancy.
  • Reduced Uterine Contractility: Helping to maintain a pregnancy.
  • Immune and Mood Modulation: Regulating inflammatory and psychological responses in the body and brain.

2. Neuroactive and Sleep-Promoting Effects (Oral Route)

When oral progesterone is absorbed, it undergoes metabolism by the liver. This process produces various breakdown products, most notably a neuroactive metabolite called allopregnanolone.

  • Brain Action: Allopregnanolone is unique because it can easily cross the blood-brain barrier and bind to GABA-A receptors in the brain.
  • Relaxation and Sleep: By enhancing the function of these GABA-A receptors (which are the same receptors targeted by tranquilizers), allopregnanolone produces a calming, sedating effect. This mechanism is why oral progesterone is often used to help promote relaxation and improve sleep for individuals experiencing hormone-related sleep disturbances.

Key Benefits of Progesterone

Progesterone is a vital hormone used in therapy for its broad effects on reproductive health, menopausal relief, and systemic well-being.

  • Uterine Protection (HRT): When used with estrogen, progesterone is essential for women with a uterus. It prevents the excessive thickening of the uterine lining (endometrial hyperplasia), which dramatically lowers the risk of endometrial cancer.

  • Sleep Improvement: Oral progesterone has a mild sedative effect, producing neuroactive metabolites that promote relaxation and help alleviate insomnia often linked to hormonal changes.

  • Reproductive Support: It is used to regulate the menstrual cycle and is crucial for preparing the uterus for implantation and maintaining early pregnancy by relaxing uterine muscles.

  • Systemic Effects: It provides ancillary benefits such as contributing to bone density and modulating the body’s anti-inflammatory and immune responses.

Potential Side Effects of Progesterone Therapy

While progesterone is generally well-tolerated, it can cause several side effects. Because oral progesterone is metabolized into sedating compounds, the most common effects are related to the central nervous system.

Common Side Effects

  • Drowsiness and Fatigue: This is the most notable effect, especially with oral capsules, which is why they are often taken at night.

  • Bloating or abdominal discomfort.

  • Headache.

  • Breast Tenderness.

  • Mood Swings or irritability.

Allergies and Formulation Considerations

An important, yet often overlooked, risk involves the formulation of commercial products:

  • Peanut Oil Warning: Many commercial brands of oral progesterone capsules contain peanut oil as an inactive ingredient, posing a significant risk for patients with peanut allergies.

  • Allergen-Free Alternative: For patients with any known sensitivities, compounded progesterone is a safe, allergen-free alternative.

  • Clinical Protocol: To ensure patient safety, always document any allergies clearly on the prescription to ensure the appropriate, non-allergic formulation is dispensed.

Who can take Progesterone?

While many think of progesterone as a female hormone, it plays a vital role in men's health as well. As part of a comprehensive hormone therapy plan, both men and women can benefit from progesterone, though men typically require it in smaller doses.

Progesterone's Role in HRT

Progesterone is a crucial component of Hormone Replacement Therapy (HRT), primarily for women who still have a uterus.

Uterine Protection

When estrogen is taken alone, it can cause the lining of the uterus (the endometrium) to thicken excessively, which increases the risk of developing endometrial cancer. Progesterone is added to counteract this effect by causing the lining to shed, offering protection against cancer.

Additional Benefits

Beyond protecting the uterus, progesterone may also be included in an HRT regimen for its potential to improve sleep.

Bioidentical Progesterone vs. Synthetic Progestins

The Difference Between Bioidentical Progesterone and Synthetic Progestins

The distinction between bioidentical progesterone and synthetic progestins is rooted in their chemical structure, which can impact how the body responds to them in Hormone Replacement Therapy (HRT).

Bioidentical Progesterone (Micronized Progesterone)

  • Chemical Identity: This form of progesterone is chemically and structurally identical to the progesterone naturally produced by a woman's body.
  • Tolerability and Safety: Because of this identical structure, it is generally considered better tolerated by the body. Observational studies suggest that, in combination with estrogen, bioidentical progesterone may be associated with a lower risk of long-term health issues (such as breast cancer and cardiovascular risk) compared to certain synthetic progestins.
  • Common Form: It is typically available as an oral, micronized capsule to aid absorption.

Synthetic Progestins (e.g., Medroxyprogesterone Acetate)

  • Chemical Identity: These are laboratory-created compounds that are similar to, but chemically distinct from, natural progesterone. They are designed to mimic progesterone's protective action on the uterine lining.
  • Side Effects and Risks: Due to their non-identical structure, synthetic progestins are metabolized differently in the body. Some women taking synthetic progestins (like medroxyprogesterone acetate or MPA) may experience a higher incidence of side effects such as mood swings, headache, or bloating. Furthermore, some large-scale studies have suggested that specific synthetic progestins may be linked to a slightly higher risk of certain adverse health outcomes, including blood clots and breast cancer.
  • Common Form: They are widely used and found in many traditional, standardized HRT preparations, including various combination pills and patches.

In summary, the choice between the two is often a balance between proven uterine protection (offered by both) and the difference in safety profiles and tolerability based on the chemical form.

Frequently Asked Questions

Feeling "Off"? It Could Be Your Hormones

If you haven't been feeling like yourself, a hormone imbalance could be the reason. Hormones are the powerful messengers that keep your body running smoothly, controlling everything from your mood to your metabolism. When key hormones like estrogen or testosterone are even slightly out of balance, it can affect your entire sense of well-being.

Do any of these changes sound familiar?

  • Shifts in your metabolism or appetite
  • Disrupted sleep cycles
  • Irregular heart rate
  • Changes in reproductive health or sexual function
  • Fluctuations in mood or stress levels
  • Difficulty regulating your body temperature

Hormone needs are different for everyone. A woman's health is closely tied to estrogen, while a man's relies on testosterone. An imbalance in either can significantly impact your quality of life.

You don't have to navigate these changes alone. See how we can help.
